How to submit a one-off forms request

This article will guide you through how to request any section of your enrollment packet throughout the year

To initiate the Forms>Request, you will first need to navigate to the Form>Templates tab

Next, you will select teh My templates filter

Here you will select from the list of forms used or provided for your school/district. Select the template you wish to request from the Guardian for your specific student

  • Here we select "Annual Physical"

Once the template is open, you will need to initiate the request by clicking on "Create batch request" in the upper right of the template

  • Note: some forms due have an Auto-approve option

Now you will begin to fill in teh required ifnromation. If sending to one student or many, you will enter the Student/Group or Section into the Subjects field

  • If you need to ensure that the form is correctable by the parent, leave the blue Standing checkbox selected. This will allow multiple submissions for the form

Now you will create the Subject of the email, as well as it contents, instruction the guardian to complete the requested forms and the optionsal Due Date

To ensure delivery notification is provided to the gaurdians, please check the "Send alert to recipients" box.

Last, click on teh Submit button after reviewing that all is correct. This will queue the platform to release the notifications to the guardians if selected.
