Start by clicking "Add allergy," which will open up a new form.
You can enter the allergen field and start typing what your student is allergic to. You may expand each type of allergen section by clicking on the pointer next to the area, such as Food.
The resulting list is scrollable. Select the appropriate allergen.
If your student's allergen is not listed, please type "other" and select the appropriate "other". You will be required to provide additional context in the Name of Allergen field.
Provide the Date of the last reaction.
Select the Type of reaction your student experiences due to this allergen.
Answer the "hospitalization" question
If "Yes", provide additional context in the required "Please explain" field.
Select the medication to be administered in case of exposure.
click on submit and continue.
