You can only submit the form once all required fields have been completed. Please review the displayed section for any missing required information as designated by an "*."
The most common items are within the Allergies, Conditions, or Medications sections. In these sections, there would have been an initial entry listed as "New Allergy," "New Condition," or "New Medication." If you still see an entry listed as "New...", click on it to verify it is blank. Then, return to the listing and click the small trash can icon next to the entry to remove.
For example, in the Medications section, you would look for an entry like the screenshot below.
