Using the multi-signature answer type in forms

How to use a multi-signature answer type to send a form request that can be signed by more than one parent or guardian.

How to Add a Multi-Signature Question to a Form Template

  1. Navigate to Forms → Templates or Forms → Packets.

  2. Open an existing template or create a new one.

  3. Click + Add question.

  4. For Answer type, select Multi-signature.

  5. In the Title field, enter text that will accompany the signature field (e.g., “Parent/Guardian Signature”).

  6. Click Save to add the question to your form.


How Multi-Signature Fields Work for Families

  • If the form is sent to multiple recipients (e.g., both parents in a non-shared household), each recipient will be prompted to sign the form.

  • If the Multi-Signature field is marked as required, the form cannot be submitted until all recipients have signed.

  • While the form is pending additional signatures, the person filling it out will see a “waiting on [Name]” message below the signature field to indicate who still needs to sign.

  • Once all required signatures are completed, the form can be submitted.

  • If only one parent or guardian in a shared household receives the form, they will be able to submit it after signing.


Notes

  • This feature is especially helpful for consent forms, permission slips, and other documents that require acknowledgment from more than one adult.

  • The signature from each recipient is logged individually and stored securely in the system.
