What are system forms?

System forms are forms created by August Schools whose content and fields cannot be changed by users at this time. The reason for this is that they contain fields that must be present on the form in a certain manner so that the information solicited on them can flow seamlessly to the student record.

The list of system forms is:

  • Providers

  • Insurance

  • Restrictions

  • OTC medications

  • Medications

  • Allergies

  • Conditions

Note: You will want to coordinate with your Customer Success Manager to update the default options on the OTC medications forms so that you solicit consent for the over-the-counter medications you actually plan on administering to students.

What do the system forms look like by default?

Providers

The form begins with the following fields:

Clicking the '+ Add provider' button allows the recipient to add information on as many providers as needed. They will need to enter the required information for at least one provider in order to submit the form.

Clicking the checkbox allows the recipient to fill out the form only after providing a comment on how care is received

Insurance

It is one single form that begins with two fields to upload the front and back of an insurance card. These fields are mandatory.

The form ends with specific fields that allow the parent to enter information on the insurance plan that will be displayed as text on the student record. These fields are optional.

Note: If these fields are not filled out, no text pertaining to the plan will be displayed on the student record; only the images of the card will be available in the Documents tab. Please let your Customer Success Manager know if you would prefer these fields be mandatory.

Restrictions

It is one single form with an optional field with multi-select checkboxes of common dietary restrictions and two mandatory questions around any unlisted restrictions and any activity restrictions.

Saying "No," to either question allows the recipient to submit the form. Saying "Yes," to either question requires the recipient to provide an explanation before being able submitting the form.

OTC medications

It is one single form that begins with a field with select checkbox options for our default over-the-counter medication options.

The form ends with a signature field that allows the recipient to digitally sign off on the over-the-counter elections they made above.

Medications

The form begins with a single question. If the recipient says 'No,' the recipient is able to submit the form.

If the recipient selects "Yes," the following additional fields appear:

For each medication, it is possible to specify Timed doses (which is optional):

Allergies

The form begins with a single question. If the recipient says 'No,' the recipient is able to submit the form.

If the recipient selects "Yes," the '+ Add allergy' field appears:

Clicking the '+ Add allergy' button allows the recipient to add information on as many medications as needed. They will need to enter the required information for at least one medication in order to submit the form.

Conditions

The form begins as a submittable form. The recipient can simply submit the form as is to indicate that there are no conditions to note.

Clicking the '+ Add condition' button allows the recipient to add information on as many conditions as needed. They will need to enter the required information for at least one condition in order to submit the form.

Explanation of the question "Which medications should be administered to Test to treat this condition?"

On the Allergies and Conditions forms, the field for this question allows the recipient to specify which of the medications that they entered on the Medications form is/are used to treat the allergy or condition.

Note: the recipient can only select these mediations after submitting the Medications form.
