Users can request that a given student’s guardians fill out a form, submit a document, all securely through the family portal.
All customers that used enrollment will find that each piece of enrollment will automatically be available as forms that can be sent to guardians. Please reach out to your Customer Success Manager so that we can start adding the forms you request most from families.
Customers using the family portal for enrollment will also have standing form requests for the constituent parts of enrollment so that families can send updated information at any time, securely. Each document that is submitted can be then reviewed for completeness before being imported to the student’s record.

What is the subject of a form request?
Similar to an encounter, the subject is the person who the care incident is about. When students are the subject, the recipient will be a guardian of the student. A practititioner will request a form from the family of the student who is the subject.
What is a standing form request?
Standing form requests are always available to the recipient. They are not bound by a due date, and can receive a submission at any time.
What does automatically approve submissions mean?
If the practitioner wants to confirm that the form that has been requested meets all specifications, they can elect to manually approve the form submission. This means there will be an approval step before the information is imported to the student or faculty card in order to make sure only approved data is on file in the system. On the other hand, to avoid these manual steps, practitioners can elect to have any submissions be automatically approved and imported to the student record.
How does a guardian access the requested form?
Families will authenticate into the family portal in the way they are already used to, either by SSO from a resource board in the student information system or by authenticating using a magic link and SMS code.

What does generate PDF form submission mean?
Any digital form can be made accessible to the practitioner at a later date as a PDF. Please select this if you would like a PDF of the record to be available after completion.
