How to produce the Student Health Summary coupled with additional physical health data and documentation

Beginning Fall 2024, the August Schools platform allows users to generate exports of one or more of the following types of information on the student record: Conditions, Allergies, Medications, Action plans, Immunizations, Encounters, Form requests, and File uploads.

Users have the ability to either particularly exclude or include individual pieces of the information from a new ‘Export student’ button that replaces the former ‘Export PDF’ button on the Student Card.

💡 This functionality still produces the Student Health Summary PDF many physical health users are familiar with but allows the users to add or remove health information on this PDF export.

When the user exports student data the following information will always be added to the PDF, if it exists on the student record:

  • Name

  • Date of birth

  • Grade

  • School

  • Pronouns

  • Gender

  • Email

  • Student ID

  • Consent form status(es) from Enrollment form request packet

  • Emergency contacts

  • Home address(es)

  • Insurance

  • OTC medication permissions

  • Outside providers

Step One: Open the Student Card.

Step Two: Select the ‘Export student’ button.

Step Three:

  1. Select ‘Single PDF’ to collate the information into a single PDF, or ‘Separate files’ to export the information into separate files per type of data.

  2. Select the ‘Zip files’ checkboxes if you would like to compress the export.

Step Five: In the ‘Chose data to include’ section, select the checkboxes for each of the following types of data to add to the PDF export:

  1. Conditions

  2. Allergies

  3. Medications

  4. Action plans

  5. Immunizations

  6. Encounters

  7. Form Requests

  8. File uploads

If you selected ‘Conditions,’ select the Conditions tab below and either:

  1. Select the ‘All conditions’ checkbox to export all the student’s condition data, or

  2. Select conditions to exclude from the export in the ‘Exclusions’ dropdown menu.

If you selected ‘Allergies,’ select the Allergies tab below and either:

  1. Select the ‘All allergies’ checkbox to export all the student’s allergy data, or

  2. Select allergies to exclude from the export in the ‘Exclusions’ dropdown menu.

If you selected ‘Medications,’ select the Medications tab below and either:

  1. Select the ‘All medications’ checkbox to export all the student’s allergy data, or

  2. Select medications to exclude from the export in the ‘Excluded medications’ dropdown menu, or

  3. Select medications to include in the export in the ‘Included medications’ dropdown menu.

If you selected ‘Action plans,’ select the Action plans tab below and either:

  1. Select the ‘All action plans’ checkbox to export all the student’s action plan documents, or

  2. Select action plans to exclude from the export in the ‘Excluded action plans’ dropdown menu.

If you selected ‘Immunizations,’ select the ‘All immunization data’ checkbox to include all the student’s immunization data, or deselect the checkbox, and:

  1. Select ‘Immunization status’ to include the compliance statuses per immunization.

  2. Select ‘Immunization table’ to include the table of immunization shot dates.

If you selected ‘Encounters,’ select the Encounters tab below and either:

  1. Select the ‘All encounters’ checkbox to export PDFs of all the student’s encounter, or

  2. Select specific encounters to exclude from the export in the ‘Excluded encounters’ dropdown menu.

If you selected ‘Form requests’ select the Form requests tab below and either:

  1. Select the ‘All form requests’ checkbox to export all the student’s form request submissions, or

  2. Select individual form requests to exclude from the export in the ‘Excluded form requests’ dropdown menu, or

  3. Select individual form requests to include in the export in the ‘Included form requests’ dropdown menu.

If you selected ‘Document uploads,’ select the Document uploads tab below and either:

  1. Select the ‘All document uploads’ checkbox to export all the student’s documents on file, or

  2. Select individual documents to exclude from the export in the ‘Excluded documents’ dropdown menu, or

  3. Select individual documents to include in the export in the ‘Included documents’ dropdown menu.

Step Six: Select the 'Confirm' button.

The export will begin to generate and will be available to download upon completion via the notification in the upper right-hand corner of the screen.
