How to: Troubleshoot Why a Supplemental Form Was Added to a Packet

Confirm whether a supplemental request rule or a parent's submission triggered the form, and resolve it correctly.

Overview

Supplemental forms are forms that only apply to select students, based on things like a health condition, demographic, or athletic participation, rather than forms every student receives. A supplemental request rule automatically adds the form to a student's packet based on how the original submission was answered. When a supplemental form shows up unexpectedly, there are two places to check: the rule that triggered it, and the submission that set it off.

This article walks nurses and sysadmins through confirming the cause of an unexpected supplemental form and resolving it the right way.

Steps to troubleshoot

  1. Open the student's packet and go to the Supplemental Request Rules tab.

  2. Open the rule tied to the form in question (e.g., the Asthma Action Plan rule).

  3. Confirm the rule is set correctly. Review the question or condition the rule depends on and confirm it matches what should actually trigger this form.

    • šŸ’” Tip: Compare the rule's conditions against a few other students who correctly received (or correctly didn't receive) this same supplemental form, to confirm the logic is behaving as expected.

  4. Confirm the parent/guardian's submission. Open the original form the parent or guardian submitted and review their answers against what the rule is checking for.

The supplemental request rule is set incorrectly

Edit the rule (for example, switch the question it's based on), then click Re-Evaluate Rules. This reruns the updated rule across all active packets, not just the one you were troubleshooting. It can take up to 30 minutes to an hour to finish processing.

The parent/guardian's submission was inaccurate

Ask the parent or guardian to resubmit the applicable form(s) with the correction. Once resubmitted, the packet will reflect the accurate information.

The parent/guardian's submission was accurate, but the student doesn't actually need the supplemental form

The nurse can archive the supplemental form directly from the student's packet.

Best Practices & Tips

  • Check the rule before reaching out to the parent. If the rule is misconfigured, it's likely affecting more than one student, so fix it at the source once rather than resubmitting forms one at a time.

  • Always click Re-Evaluate Rules after an edit. This makes sure every active packet, not just the one you're troubleshooting, reflects the corrected logic. Give it up to an hour to finish processing before checking the result.

  • Archiving is for accurate-but-unnecessary forms only. If the submission itself was wrong, please submit a corrected version instead of archiving. Archiving doesn't fix inaccurate data on file.
