How to set rules for supplemental form requests within a form request packet

A Form Request Packet is two or more form requests sent together to a recipient for a given (set of) student(s). In some case, such as enrollment, some of the forms requests in a form request packet will not apply to every student that received the form request packet and therefore should be supplemental. Supplemental request rules exist to display certain form requests within a form request packet for completion only when the student meets certain conditions. Most of the time, these conditions are based on submitted responses to other form requests in the same form request packet.

Note: Once created, supplemental request rules will only be editable by the creator. Therefore, the user designated to create them should be available for any additional edits until the Form Request Packet is finalized and sent out.

How to Create Supplemental Request Rules

Step One: Open the form request packet from the Forms > Packet page.

Step Two: Identify which form request(s) should be supplemental.

  • In this case, we want the Serious allergies action plan to be supplemental so that only the students whose parents said they had a serious allergy should have the form request in their packet.

Step Three: Toggle the 'Supplemental?' button from 'No' to 'Yes.'

Step Four: Select the 'Supplemental request rules' tab at the top of the form request template.

Step Five: Select the '+ Add rule' button at the bottom of the tab.

Doing so will had an entry for a rule with the default name 'New supplemental request rule.'

Toggle the entry down to expose additional fields and information pertaining to the rule.

The first rule that is added by default tells the platform to look at the form request packet you are in. Please do not edit or delete this first rule in all cases.

The second rule that is added by default tells the platform to, if you are using another form request to base the rule, make sure that the form request has been submitted first before applying the rule (which is the best practice, as that will ensure that the recipient's submitted, and not draft, response is being used). As such, please do not edit or delete this second rule, in most cases.

Step Six: Add a sub rule to the Form requests rule group to identify on which submitted form request response value to base the rule. The sub rule will default to a 'Submitted at' value.

Step Seven: Change the value of the sub rule to 'Form instance.'

Step Eight: In the new field that appears, select the submitted form request response value you want to use to set the rule.

  • This response value will be a question on the desired form request you want to use.

  • In this case, we want to use the answer to the General health form question 'Does %{student.first_name} have any severe or life-threatening allergies?' to trigger the Serious allergies action plan form.

Step Nine: Select the answer to the question that should trigger the form.

  • In this case, we want the recipient to select 'Yes' to the question 'Does %{student.first_name} have any severe or life-threatening allergies?' to trigger the form.

Step Ten: Select the 'Save changes' button to save the supplemental request rule.
