How to: Create a New Form Request Template

A Form Request Template is the layout of the form request you will send out regarding (a) student(s). The template specifies what information the recipient will see and how the recipient will action on the request.

Step One: Navigate to the Forms > Templates page

If the page does not appear on your left navigation pane, you may use your URL by adding '/forms/templates' to the end of it.

For example, if your URL is school.augustschools.com, you can navigate to school.augustschools.com/forms/templates.

Step Two: Select the 'New Template' button

It is a blue button located toward the top left of the page.

You will be able to choose from either a previous template you created or a default template created by August Schools. For the time being, you can always create a new request from scratch.

Step Three: Give the template a title

Click into the 'New template' hyperlink towards the top left of the page to delete that text and enter your own.

Step Four: Set the visibility of the form request

You are able to specify which users who share one or more roles as you should see both the request itself as well as the response(s) to it.

To select, click into the text box and choose the role(s) from the dropdown menu.

Step Five: Add questions to your template

Adding questions to your form allows you to specify what information the recipient sees and how they should respond.

To add a question, select the blue '+ Add question' button.

Doing so exposes a window that allows you set the following fields on the answer:

  • Answer type

  • Title

  • Description

For each question, you will need to specify the answer type. Answer type specifies that type of information your are adding to the template, such as a multiple choice question that allows multiple answers, to a text block that simply provides information to the recipient.

Tip: Text block is the best answer type to use when you have large amounts of text to share with the parent. This answer type provides several text formatting options and the ability to make this field a pop-up to save initial space when the recipient opens the form!

For most questions, you will need to specify the title. The title allows your to dictate what question to ask, or what information to provide. For example, if you are asking a question to the recipient, set the title to be the actual question you want answered!

For each question, you can add a description. The description provides more information to the recipient about the question, such as sub-text. When the question is a text block, the description displays the text in that block. You can use the text formatting buttons on this field to update the visual presentation of this description text.

For most questions, you can make the question required or optional. This is achieved by selecting or deselecting the 'Answer required' checkbox when present.

Step Six: Send the form request out via a batch request

Once you have configured your template to ask and display all the necessary information, you can send it out by selecting the 'Create batch request' toward the top right of the page.

You will then follow the existing way of creating a batch request using this button, as it is the same button and workflow as on the Forms > Requests page! For more information on this, refer to this help center article.

Step Seven: Review the form request on the Forms > Requests page

When you send the batch request out, you will be able to track its submission status and information on the Forms > Requests page, specifically on the 'All batches' tab.
