How to Start a Form Template Using an Existing Document

Many times, you may need to recreate a physical or electronic document you into a form templates that your recipients (e.g., parent, faculty) can fill out within the August Schools Portal or Platform. While you can follow the steps outlined in this hyperlinked article to recreate the form template from scratch, you may follow the following steps in this article to get a head start by uploading the (digital copy of the) document you have.

Step One: Navigate to the Forms > Templates page

If the page does not appear on your left navigation pane, you may use your URL by adding '/forms/templates' to the end of it.

For example, if your URL is school.augustschools.com, you can navigate to school.augustschools.com/forms/templates.

Step Two: Select the 'Import form templates' button.

Step Three: Upload the file that contains the document you wish to replicate by either clicking, or dragging the file to, the gray upload area.

We recommend that your upload one document to replicate at a time, as you will need to make some adjustments to each file that is imported before using the file template.

Step Four: Click the 'Import 1 file' button.

You will see that your file has been successfully uploaded both on both a pop-up message and by the presence of a hyperlink to the file under the gray upload area.

Step Five: Wait for the platform to conduct a first pass at translating your document into a form template.

You will see the status of this work both on a pop-up progress window and in the table below, where there will be a row for your uploaded file and a column for the status of the import and the progress of the import.

When the status of the import changes to 'Needs inspection,' you will then be able to review how the platform created the form template and make any revisions.

Step Six: Click into the row for your imported file to review how the platform created the form template.

By doing so, you will first be brought to page for the import itself. Click into the first row to start your review. Note: Although we recommend uploading one document at a time, if you do upload multiple documents, there will be a row for each import who file template you will need to approve.

Step Seven: Review each question that the platform has created for the form template from your file.

Each question will be available in a scrollable list to the left of the preview of the document. To edit each question, select the 'Edit question' button. For more information on editing questions, refer to step five of this hyperlinked article and review this hyperlinked article.

If the platform missed a question, or you want to add an additional question upon review, simply select the '+ Add question' button.

Step Eight: Select the 'Approve' button to approve the questions to be added to the form template.

Doing so will freeze the question fields on the import page and display an 'approved' status next to the tile of the form template.

Your new form template will be added to the end of the table of templates on the Forms > Templates page.

Reminder: You will be able to continue editing this form template by clicking its row on the table of form templates on the Forms > Templates page.
